Referring a Patient

OptionCare makes the referral process easy.

For Home Infusion Services, Respiratory Therapy / Home Medical Equipment, and Other Services such as Home Health and Hospice:

1. Collect the following information:
   • Patient demographics
   • Patient diagnosis
   • Patient history and physical
   • Physician orders
   • Insurance information
   • Most convenient way to contact you
2. Use the convenient Site Locator to find your local OptionCare office. Then, either:
   • Call your local OptionCare office–just one phone call is all it takes.
   • Or
   • Download the Referral Form and fax it to your local OptionCare office.
