Each patient is assigned a primary care manager who coordinates communication and encourages compliance with the prescribed treatment plan. A toll-free assist line provides complimentary clinician support 24 hours per day, 7 days a week. Through one-on-one assessments and review of each patient's status, OptionCare's Condition Management Program provides an individualized health management action plan based on the latest national guidelines to help prevent treatment complications and maximize treatment outcomes.
